Types of Chaperones Services in Partner Programs

Chaperone service level is keyed to client needs. In consultation with the Client, the provider or agency-trained professional human services Coordinator decides which level applies in the individual situation and arranges the transportation, Chaperone, date, and time of service.  Options include:

 

  • Full door-to/through-door coverage, with the Chaperone going to the Client's house each way.

  • One-way door-to/through-door coverage, usually for the trip home.

  • Ride monitored telephonically, with a Chaperone accompanying remotely throughout.

  • Meeting the Client at the healthcare facility to navigate through the hospital and stay with the Client if necessary. Depending on the arrangement, the Chaperone may stay through the appointment or return to pick up at the end of the appointment.

Additional Supported Chaperone Services in Partner Programs

Wellness Check-ins with phone calls to Clients with whom a Chaperone has established an in-person Chaperone/Client connection.

  • Help Schedule Medical Appointments. 

  • Vaccine Appointments. 

  • Reminder Calls.

  • Follow-up Check-in calls post appointment or procedure apart from Doctor office calls

  • Notes Client Needs: If the Agency or Hospital Volunteer Coordinator learns of situations, either through the client calling in for a Chaperone, OR the Chaperone noting the need to talk and travel with the Client, in which a client requires assistance beyond transportation, such as mobility support or translation services, the Coordinator will refer the client to the appropriate services. Because the Chaperone actually travels with the client, sees their home environment, and settles the Client in on a return trip, the Chaperone is in a unique position to speak with and notice aspects of the living situation of the Client.

Chaperone In-Home Service for Telehealth Assistance

  • Adaptation to changing healthcare practices with in-home Chaperone service for telehealth assistance, a practice that grew during the pandemic and is now an established practice with most healthcare providers.

  • Chaperones can take pre-set tablets to Clients at their homes to assist them with telehealth visits or assist clients on their own computers by helping them set up provider telehealth sites.

  • Chaperones can assist clients during the telehealth appointment by taking notes or making sure the client understands the information a doctor is imparting, with hospital and/or provider permission, HIPAA protection, and at the client's request.
